[cam-users] MPI files

I would recommend setting the environment variables INC_MPI and LIB_MPI
rather than editing the Makefile. Since the Makefile is generated by
configure, editing it by hand must be done every time a new configuration is
built. This is very error prone.
The commandline options -mpi_inc and -mpi_lib can also be used, and those
settings will take precedence over the environment settings. But in
general setting the environment variables in your startup file
(e.g. .cshrc) is the easiest method.
