update most of the top level html files to use the actual
format of namelist that a user would add to the input.nml file
(used to be the format for declaring a namelist in the f90 code).
update the table that contains the namelist value explanations to
avoid duplicate locations of the default values.  updated a few of
the .nml files to match the defaults in the code.
